If you’d rather ask something before scheduling, leave your details here and I’ll get back to you during weekday hours. This form deliberately doesn’t ask about your health — save those details for your visit.

Give at least one way to reach you — email or phone.

Please don’t include health details here — this form is for getting started and scheduling only. Clinical questions belong in your visit or the secure patient platform.
