Adult ADHD Evaluation, Diagnosis & Treatment — Telehealth in Maine

Comprehensive ADHD Evaluation

Ninety minutes with a board-certified psychiatric nurse practitioner, plus Creyos cognitive testing and MindMetrix. Not a five-minute quiz that always says yes.

How the evaluation works

Every evaluation follows the same three steps, and every evaluation includes objective testing. Structured, evidence-based measures inform every diagnosis, so the answer isn’t a guess.

Before your visit

Intake forms, MindMetrix, and Creyos cognitive testing, completed from home on your own schedule.

The 90-minute evaluation

A structured clinical interview by secure video. Results are not reviewed in this session.

Results & treatment alignment

A 30 to 45 minute follow-up to go through everything the testing showed and decide next steps together.

The clinical interview

The center of the evaluation is a 90-minute structured conversation about your life, from childhood forward. I’ll ask about school and how learning felt, work and how you manage deadlines and details, relationships, sleep, mood, and the coping systems you’ve built over the years.

What I’m looking for is a pattern: whether attention and executive-function difficulties began in childhood, show up across more than one part of your life, and cause real impairment, which are the core criteria for an ADHD diagnosis. I’m also looking for what else could explain the picture, because anxiety, depression, sleep problems, and burnout can all look like ADHD.

Validated rating scales

The CAARS (Conners’ Adult ADHD Rating Scales) are standardized questionnaires that compare your experience with large samples of adults. There are two versions:

  • CAARS self-report: your own ratings of your symptoms. This is usually completed between the evaluation and your follow-up visit.
  • CAARS observer report: completed by someone who knows you well, like a partner, parent, or close friend. It adds an outside view, since we all see ourselves imperfectly.

MindMetrix: a broad look before we meet

MindMetrix is a comprehensive psychiatric pre-screening assessment you complete before your evaluation. Rather than screening for ADHD alone, it looks for elevated likelihood across more than 60 conditions, along with how your symptoms affect day-to-day functioning and how severe they are.

It helps me in two ways. It means less time on preliminary questions during your visit and more time on what matters. And it helps surface co-occurring or look-alike conditions that a narrower screen could miss. MindMetrix is a prescreen, not a diagnosis; it guides where I look closer.

Creyos: objective cognitive testing

Creyos is a set of short, game-like computer tasks completed from home. It measures cognitive functions that ADHD commonly affects, including attention, working memory, processing speed, and response control, and compares your results with norms for people of your age.

Most of an ADHD evaluation relies on what you remember and report. Creyos adds objective data alongside that history, so the conclusion rests on more than self-report. Like every test, it informs the diagnosis rather than making it on its own; there’s no pass or fail.

Measurement-based care

The same structured measures used during your evaluation are repeated over time in follow-up care, so progress is tracked with real numbers rather than impressions alone.

If the results don’t show ADHD. Ruling ADHD out can be just as valuable as identifying it. The evaluation may show that attention difficulties are more closely related to anxiety, depression, sleep disruption, stress, or another condition, or that ADHD is present along with something else. Whatever the outcome, you’ll leave with a clearer understanding of what’s happening and what may help.

Cost

The complete ADHD evaluation is $800, paid in three parts: a $250 booking deposit, a $350 evaluation fee charged 24 hours before your evaluation, and $200 for the results visit. Full details are on the Fees & Payment page.
